1. Please visit the Registrar's Office website to review the regulations, print the Leave of Absence application form, and prepare the required documents.

2. Schedule appointments with your academic advisor, program director, and program office in sequence to obtain their signatures.

3. Submit the completed application form to the Registrar's Office.

Important Notes:

1. You are required to complete all the Leave of Absence application procedures on your own. The program office will not accept applications on behalf of students.

2. Please be aware that your academic advisor, program director, and program office may have holidays or vacation periods. It is advisable to contact them in advance to schedule appointments.

3. Please pay attention to the Leave of Absence application deadline and tuition refund schedule. Late applications will not be accepted.
